Good afternoon everyone this is Jane from the Great Plains Food Bank I just kind of want to run through our senior commodity food application that we have for the seniors um one of the biggest things that I notice is they don't always know how to fill it out correctly, and I thought it would be some great information for you as somebody who is either volunteering at the food pantry or helping out that if they had any questions that you would know the specific pieces of information that I need in order to complete their application so this first page here is basically just the cover letter on it kind of explains what I'll need to process the application and then also here at the bottom it talks about where you can return the application to, so it can be mailed back to me, it can be scanned it can be emailed it can be faxed on whatever is easiest for you, and it's easier for the clients to also see where they can send it back to as well most obviously mail it, but you have the food pantry if you want to kind of step in and help them and get it to me faster if the distribution is coming up that is fine with me too so here is the first page of the application obviously making sure that their name their address the city state county and telephone number or filled out the home delivery and pickup um if you are the blue pantry, and you're in charge of helping with the senior commodities it really doesn't matter what they end up picking out here on they can pick either home delivery or pickup it doesn't matter on whatever you as a food pantry are able to do make sure they fill this Hispanic or Latino section and obviously their race for the household members this includes the actual senior that's in the household as well so whether they have won or need more spaces just make sure that the amount of household members that they have all their name and date of births are all sold out because that kind of goes into effect of how they would be approved for the program since it is based on age and household size plus income this part right here it's whatever the client would like to choose they can either give us access or not um there is no wrong or right choice for what they decide to pick and then obviously making sure that their signatures here at the bottom and dated, and then I will sign and date there as well once I receive it the second page here gets missed a lot, but this is one of the biggest things is you know once we make sure that they're over the age of 60 we also need to make sure that their income lines up with the amount of household members that they have so doesn't matter what any kind of income they receive that is monthly, or bimonthly it needs to be recorded here on if they end up doing the yearly that's okay too because I can always divide it by 12 to get the monthly amount but make sure that the number of household members are all here and make sure that any kind of income that they receive monthly or bimonthly is recorded on this...
